Thank you for your interest in pursuing treatment with me.  I am truly passionate about the care I provide my clients and am invested in helping them achieve their goals.  By seeking treatment you are choosing to take ownership of the healing process whether it is to recover function or relieve pain.

Due to the numerous insurance companies that serve our area I am able to participate with a limited number. (Coastal Health/Community Health and Triwest/Tricare and Blue Shield). Under "patient forms" I have provided you with an outline of questions to ask your insurance company to determine coverage.  Be sure to tell them that I provide a specialty service that is not currently offered by any other therapist on the Monterey Peninsula.  This will often allow for better coverage.  You are welcome to give them my name and contact information to answer any further questions or clarification required.

At the time of evaluation I will give you a standardized insurance claim form and your itemized invoice. These will include all codes and information usually required to file a claim. 

This process has worked well for my patients and removes the usual barriers to treatment. You will know up front what the fee for service is and there will be no surprise bill at the end of our treatment.
